Impact of the French top-down management of the COVID-19 pandemic on residential care homes

Résumé

This chapter examines the impact of the highly centralised management of the COVID-19 pandemic on residential care homes in France. The French response, characterised by a top-down governance structure, revealed significant challenges, including a lack of coordination between central and local authorities, inadequate integration between health and social care sectors, and the hospital-centric nature of the healthcare system. These systemic issues led to insufficient monitoring of the epidemic within care homes and delays in delivering protective equipment and care protocols. Despite the challenges, the crisis prompted innovations such as home hospitalisations and geriatric networks, which helped mitigate some initial failures. The chapter highlights the critical need for structural reforms in staffing, resource allocation, and local governance to address long-standing weaknesses in the French long-term care system.
