Neurocognitive processes of food addiction in patients with severe obesity: A functional magnetic resonance imaging (FMRI) exploration study

Rationale: Food addiction (FA) could modify the management of obese patients. fMRI may help to identify the cognitive processes underlying FA. Aims: (i) to identify by fMRI specific responses in brain regions involved in hedonic and motivational processes (striatum), and cognitive control (prefrontal cortex) according to FA severity; (ii) to identify whether FA severity is related to anxiety and/or depression. Methods: Design: prospective, observational, monocentric study including grade 2 or 3 obese women with FA or not. Each patient underwent one fMRI session that included a sequence of images during a food cognitive task and a sequence during a resting-state condition. Anxiety-depressive symptomatology and body mass index (BMI) were assessed. The relationship between FA severity and other variables was assessed using ordinal logistic regression. A generalized linear model was used to detect brain responses. Results: Forty female patients, age (mean±SD) 36.7 ± 12 years, BMI, 39.2 ± 3, were included, 60% of them suffering from FA. The FA severity was associated with more frequent anxiety symptomatology (odds ratio, 4.93; 95% confidence interval [1.25; 19.60]; p=0.04). In the food task condition, FA severity was associated with decreased brain activation in the bilateral putamen as well as anterior cingulate cortex (ACC) (voxel: uncorrected p<0.001). In resting-state condition, FA severity correlated negatively with functional connectivity (FC) between the ACC and dorsal striatum (voxel: uncorrected p < 0.001; cluster: p-FDR < 0.05). Conclusion: Brain profiles of obese patients were heterogeneous related to FA severity. The brain areas that had differences in activation were those involved in emotional regulation and cognitive control and motivational processes. FA severity is associated with impaired cortico-striatal FC in patients with severe obesity.
