Acceptability of an adapted physical activity program supervised by videoconferencing in adolescents with anorexia nervosa: a pilot study.

Introduction: Anorexia nervosa (AN) primary affects adolescent girls and is often associated with physical and psychological disorders. Medical management requires the use of various non-pharmacological therapies, such as adapted physical activity (APA), in order to reduce main symptoms of anorexia (dieting, low weight, etc.) and its associated disorders. However, interventional research in this area remains limited and sparse. Moreover, the scientific literature does not report any evaluation of the feasibility, acceptability and effectiveness of a supervised APA intervention by videoconference in the homes of AN patients. The interest of this device would be to promote therapeutic continuity in outpatient care. The primary objectives of APAREXIM’Pilot study were to assess the acceptability of an APA program (2x1h per week for 8 weeks) by videoconference and its effect on the main symptoms of AN. The secondary objectives were to assess its effects on sleep-wake cycle, and on mental and physical health. Method: 14 adolescent girls from the pediatric psychiatry department of Caen University Hospital were randomly assigned to the control group (n = 7) and the APA group (n = 7), and completed pre-program (T1) and post-program (T2) assessments. Results: The score of APA acceptability questionnaire via video-conferencing (TAPQ) was very high and consistent (13.28/16 ±3.8). A significant (p<.05) increase was observed for sleep efficiency in APA group, trunk endurance extensors and elbow extension strength. No significant effect was observed on main symptoms of AN and mental health. Conclusion: Although sample size was small, primary objectives were met since APA by video-conferencing was well accepted and had no deleterious effect. Concerning secondary objectives, sleep and physical health appeared to be improved. Nevertheless, these promising results will require expansion of this study to a large sample size to achieve a satisfactory level of evidence.
