Therapeutic letting go : a specific mode of action of spa treatment in mental health ?
Le lâcher-prise thérapeutique : un mode d'action spécifique de la cure thermale en santé mentale ?

Spa treatment has demonstrated its effectiveness in the treatment of generalized anxiety and
depressive disorders, insomnia and benzodiazepine withdrawal. Institutional models in psychia-
try seem insufficiently adapted to the management of mental disorders: hospitalization appears
too restrictive and outpatient medicine does not allow for support and distancing over a sufficient
period of time. The spa centre appears to be an adapted, integrative model, which allows the
implementation and combination of several complementary therapeutic actions. This update pre-
sents the knowledge acquired on the effect of therapeutic letting go in spa treatments. The early
relaxation encountered during the treatment appears to be a predictive factor of the clinical
improvement induced by the spa treatment. The state of letting go following hydrothermal treat-
ments could correspond to the realization of a mental experience of relaxation with modification
of the flow of consciousness. After encouraging preliminary work, the LetGoTherm study eva-
luates the evolution of the psychosomatic state and therapeutic letting go of patients suffering
from mental disorders and benefiting from three weeks of spa treatment. The scales used are the
validated Eqfc scale, the Saujon Mini Scale, the HAD scale, the ISI scale and the Saujon psycho-
somatic state scale. The therapeutic letting go induced by the spa treatment appears to be a deter-
mining factor in allowing patients to become more available and more capable of changing their
psychophysiological state.
La cure thermale a démontré son efficacité dans le traitement des troubles anxieux généralisés
et dépressifs, de l’insomnie et le sevrage des benzodiazépines. Les modèles institutionnels en
psychiatrie semblent insuffisamment adaptés à la prise en charge des troubles mentaux que ce
soit l’hospitalisation trop contraignante ou la médecine ambulatoire qui ne permet pas l’étayage
et la mise à distance sur une durée suffisante. La cure thermale apparaît être un modèle adapté,
intégratif, qui permet la mise en action et la combinaison de plusieurs actions thérapeutiques
complémentaires. Cette mise au point présente les travaux réalisés sur l’effet du lâcher-prise
thérapeutique en cure thermale. La précocité de la détente rencontrée au cours de la cure appa-
raît être un facteur prédictif de l’amélioration clinique induit par la cure thermale à orientation
psychosomatique. L’état de lâcher-prise consécutif aux soins hydrothermaux pourrait
correspondre à la réalisation d'une expérience mentale de relaxation avec modification du flux
de conscience. Après des travaux préliminaires encourageants, l’étude LetGoTherm évalue
l’évolution de l’état psychosomatique et du lâcher-prise thérapeutique de patients souffrant de
troubles mentaux et bénéficiant de trois semaines de cure thermale. Les échelles utilisées sont
l’échelle validée Eqfc, la mini-échelle de Saujon, l’échelle HAD, l’échelle ISI et l’échelle d’état
psychosomatique de Saujon.
Le lâcher-prise thérapeutique induit par la cure thermale apparaît être un facteur déterminant
pour permettre aux curistes de devenir plus disponibles et plus aptes à changer leur état psycho-
physiologique.
