Roles, missions and training of French state-qualified nurse (RN) in spa establishment

The presence of French state-qualified nurse (RN) is constant within spa establishment. The aim
of this survey is to identify the roles, missions and training of nurses in French spas. A self-admi-
nistered and anonymous questionnaire was sent by e-mail in June 2023 to 89 establishments in
France that are members of the Conseil national des établissements thermaux (Cneth), with a
deadline for responses of 31 August 2023. The 34 respondents were mainly women (94 %), aged
between 40 and 65 (71 %). Questions were asked about the functions of nurses in spa establish-
ment: the results show coordination missions (44%), an exclusive nurse activity (35%), care
supervision, management of the medical centre or responsibility for hygiene. The RNs state that
they play a coordinating role between the patient, the spa doctor and the care team (88 %).
Regarding job adaptation training on recruitment, 74 % declare that they have received no training
in spa therapy, 71 % report a need for training, 84 % respond that an introduction to spa therapy
should be included in the initial training curriculum, 32 % in that of advanced practice nurses
(APN). Concerning advanced practice in a spa, 58 % are in favor of it and propose as specific
missions: adapting or modifying spa treatments, organizing therapeutic education activities,
carrying out the second consultation according to a pre-established protocol. Concerning the
reasons for working in a spa, 58 % of the nurses said they wanted to work independently and were
interested in the spa, 55 % said they wanted to work in a pleasant environment, 50 % said they
wanted to work at a fixed pace and with fixed hours, 47 % said they wanted a more global
approach to the patient, 44 % said they wanted to work close to home, 35 % said they had been
burnt out in the past and 29 % said they had been personally dissatisfied in the past. Most respon-
dents (90 %) described their professional situation positively. It should be noted that 27 % said
that they were tired because of their working conditions. This study shows that spa nurses have
chosen this profession out of an interest in the spa industry, but also for a personal and professional
balance. The spa nurses carry out acts that may fall within the scope of advanced practice. The
development of APNs in spas would enable the work already carried out to be professionalized,
and certain tasks to be developed with a view to freeing up medical time and improving the care
of patients, while at the same time offering these nurses an opportunity to professional practice.
La présence d’une infirmière diplômée d’état (IDE) est règlementaire au sein d’un établisse-
ment thermal. L’objet de cette enquête par questionnaire est de recenser les rôles, missions et formation des IDE dans les centres thermaux français.
Un questionnaire auto-administré et anonyme a été adressé par courriel en juin 2023 à 89 éta-
blissements du territoire national adhérents du Conseil national des établissements thermaux (Cneth), avec une date de réponse limite au 31 août 2023. Les 34 répondants sont majoritaire-
ment des femmes (94 %) dont la classe d’âge se situe entre 40 et 65 ans (71 %).
Ont été questionnées les fonctions de l’IDE dans l’établissement thermal : les résultats objecti-
vent des missions de coordination (44 %), une activité exclusive d’IDE (35 %), une activité en éducation thérapeutique pour la santé, l’encadrement de soins, la direction du pôle médical ou
la responsabilité de l’hygiène. Les IDE déclarent assurer un rôle de coordination entre le curiste,
le médecin thermal et l’équipe de soins (88 %).
Concernant la formation d’adaptation à l’emploi lors l’embauche, 74 % déclarent n’avoir reçu
aucune formation en thermalisme, 71 % rapportent un besoin d’une formation en thermalisme,
84 % répondent qu’il faudrait inclure une initiation au thermalisme dans le cursus de formation
initiale, 32 % dans celui d’infirmiers en pratiques avancées (IPA).
Concernant la pratique avancée en établissement thermal, 58 % y sont favorables et proposent
pour missions spécifiques : l’adaptation ou la modification de soins thermaux, l’organisation des activités d’éducation thérapeutique, la réalisation de la seconde consultation selon un proto-
cole préétabli.
Concernant les motivations d’un exercice au sein d’un établissement thermal, 58 % des IDE
répondent le travail en autonomie et leur intérêt pour le thermalisme, 55 % le cadre de vie, 50 % le rythme de travail et les horaires fixes, 47 % l’approche plus globale du patient, 44 % la proxi-
mité avec le domicile, 35 % une situation antérieure d’épuisement professionnel, 29 % une situation antérieure d’insatisfaction personnelle. La majorité des répondants (90 %) qualifie leur
situation professionnelle positivement. À noter que 27 % déclarent la fatigue inhérente aux
conditions de travail.
Cette étude montre que les IDE travaillant en station thermale ont choisi ce métier par intérêt
pour le thermalisme mais également pour un équilibre personnel et professionnel. Les IDE ther-
males accomplissent des actes pouvant relever de la pratique avancée. Le développement des
IPA en établissement thermal permettrait de professionnaliser le travail déjà accompli, de déve-
lopper certaines missions en vue de libérer du temps médical et d’améliorer la prise en charge
du curiste.
