Sustaining speech development in French-speaking children fitted with CI: a comparative study on different intervention strategies

The cochlear implant, being in constant technological evolution, requires an evidence-based adaptation of speech therapy practices used to support speech development in deaf children. We have therefore developed a battery of tests designed to evaluate speech perception and production in these children, in order to (i) identify the specific difficulties that French-speaking children with CI are faced with, and (ii) compare different therapeutic approaches to oral communication support. These tests, which include picture naming, non-word repetition, syllable repetition, lexicality judgment, and phonological awareness tasks, are administered to French- speaking children with cochlear implants aged 4 to 11 years, and compared to a large cohort of typically-hearing children. Age at implantation, duration of implant use, multilingualism, types of hearing devices, etc. are taken into account to explain the variability of their speech development. Our study specifically focuses on two approaches with diametrically opposed theoretical foundations, Cued Speech, which is resolutely multimodal since it is based on the use of manual gestures to supplement auditory and visual information, and Auditory Verbal Therapy, which is oriented towards monomodal training in auditory perception. These two practices are also implemented very differently in France, since Cued Speech is recommended by the health authorities and disseminated by professionals, whereas Auditory Verbal Therapy is promoted and used by parents themselves, and disseminated mainly through social networks. Our study on phonological development is thus completed by a survey on the uses of the various communication tools for deaf children. We highlight the need for a wider dialogue between users, families of users, professionals, and researchers on these issues.
