Biomarkers of genotoxicity in medical workers exposed to low dose ionizing radiation: systematic review and meta-analyses

Introduction Medical staff are among the workers with the highest occupational exposure to ionizing radiation (IR) over the world. Chronic exposure to low dose of IR may result in DNA damages, then leading to cancers. This review aims to provide both an overview and a classification of genetic endpoints most likely to be used as proximal signs of disease and as long-term predictors of clinical events in chronic IR-exposed healthcare professional studies. Methods A systematic review of the literature was performed to retrieve relevant studies from 2000 to 2020, studying chromosomal biomarkers in medical workers exposed to protracted occupational IR low doses. It was supported by meta-analyses giving a pooled effect size from standardized mean differences between IR-exposed and unexposed workers for several endpoint frequencies. Results The selection procedure retrieved 71 studies. Dicentric, acentric fragment and MN frequencies were significantly different between IR-exposed and unexposed workers (θpooled = 1.41; 95% CI 0.97-1.86 for MN), which was not the case for ring chromosomes, proliferation indexes, and nucleoplasmic bridges. Translocations, sister chromatid exchanges, comet tail length, comet tail moment, and comet score are strong candidates for biomarkers of IR exposure (θpooled = 12.73µ; 95% CI 8.70-16.75 for tail length). Conclusions This review confirms the relevance of some biomarkers whose number or length proved to be significantly different between IR-exposed and non-exposed workers. Some endpoints require further studies to confirm that their interest. The integration of genetic biomarkers in epidemiology may provide important insights regarding exposure, effects and susceptibility to IR.
