Anti meningococcal B vaccination practices by Bexsero® : prospective observational study of liberal general physicians installed in Picardie

Introduction: The non-reimbursement of Bexsero® was one of the factors limiting the use of Bexsero® in the prevention of invasive meningococcal B infections. This vaccine is now reimbursed by social security at 65% in the target populations. The objective of our work was to evaluate the impact of this reimbursement on its prescription. Study design: This was an epidemiological study by sending questionnaires by post and online to 400 general practitioners established in Picardie between March and June 2023. Results: The response rate was 35%. 67.9% were in favor of reimbursement for this vaccine. 95% felt more comfortable offering vaccination to the target patient population and 62.9% offered it to them systematically.85.7% estimated that the number of their vaccinations had increased, and 40.7% considered vaccination coverage sufficient. The patient population was rather favorable at 60.7%. 85% of doctors questioned were in favor of making it compulsory. Discussion: Most physicians were satisfied with the reimbursement of Bexsero®. The favorable arguments were the severity of the illness and the after-effects. The obstacles to prescribing were the complexity of the vaccination schedule, and a non-obligatory nature of the vaccine which may lead to its effectiveness being underestimated. Conclusion: Our study showed that the reimbursement of Bexsero® had a positive impact on its prescription among these doctors but vaccination coverage is not yet optimal, undoubtedly linked to the recent nature of the reimbursement.
Introduction : le non-remboursement du Bexsero® était un des facteurs limitant l'usage du Bexsero® dans la prévention des infections invasives à méningocoque B. Ce vaccin est désormais remboursé par la sécurité sociale à 65% dans les populations cibles. L'objectif de notre travail était d'évaluer l'impact de ce remboursement sur sa prescription. Matériel et méthode : il s'agissait d'une étude épidémiologique via envoi de questionnaires par voie postale et en ligne à 400 médecins généralistes installés en Picardie entre mars et juin 2023. Résultats : le taux de réponse était de 35%. Ils étaient 67,9% à être favorables au remboursement de ce vaccin. 95% se sentaient plus sereins de proposer la vaccination à la patientèle cible et 62,9% le leur proposaient systématiquement. Ils étaient 85,7% à estimer que le nombre de leurs vaccinations a augmenté, et 40,7% jugeaient la couverture vaccinale suffisante. La patientèle était plutôt favorable à 60,7%. 85% des médecins interrogés étaient favorables à le rendre obligatoire. Discussion : la plupart des médecins étaient satisfaits du remboursement du Bexsero®. Les arguments favorables étaient la gravité de la maladie et les séquelles. Les freins à la prescription étaient la complexité du calendrier vaccinal, et une non obligation du vaccin qui peut laisser sous-estimer son efficacité. Conclusion : notre étude a montré que le remboursement du Bexsero® a impacté positivement sa prescription chez ces médecins mais la couverture vaccinale n'est pas encore optimale, sans doute en lien avec le caractère récent du remboursement.
