              <p>Radiopharmaceutical therapy (RPT) represents a significant intersection of nuclear medicine and targeted therapy, using radioactive substances to deliver ionizing radiation directly to diseased cells while sparing surrounding healthy tissue, in order to treat various diseases, predominantly cancer. Over the past few years, it has emerged as a groundbreaking therapeutic modality, leveraging the unique properties of radionuclides to deliver molecularly targeted therapy with high precision and transforming the landscape of oncology and personalized medicine. The development of radiopharmaceutical therapy reflects decades of advances in nuclear medicine, chemistry, and cancer biology. It represents the convergence of basic scientific research and clinical application, aimed at improving outcomes for patients with tumors that are difficult to treat with conventional therapies. Significant progress continues to be made, and there has been considerable expansion in the available therapeutic radionuclides and the pathologies which can be managed by them. This chapter will deal with the underlying principles of radiopharmaceutical therapy, clinical applications, ongoing research, challenges and future prospects in the field.</p>
