Functional and MRI study of corpus callosum in relapsing-remitting multiple sclerosis.

BACKGROUND: Atrophy and lesions of CC and peri-ventricular areas are frequents in RRMS, so in early stage of the disease (Andouin et al., 2007). However, little is know about the real functional and cognitive impact of these morphological disorders (Pelletier et al., 2001, Warlop et al., 2008), with discordant results, specifically about impact of CC atrophy and interhemispheric communication. OBJECTIVE: To investigate the relation between damage in the CC, the cognitive performances and interhemispheric communication task for RRMS patients. METHODS: 34 right-handed RRMS patients and 22 matched controls were compared in an extensive neuropsychological assessment and on four interhemispheric transfer tasks of auditory (verbal dichotic listening), somesthesic (crossed tactile finger localization), visual (crossed matching visual) and motor (Poffenberger paradigm) informations. A MRI scan appreciated the extent of white-matter changes and the degree of total and patients regional callosal atrophy. Means of regional callosal atrophy were delineated on four medial sagittal slices and referred to as the genu, the anterior and posterior body, and the splenium of CC. RESULTS: RRMS patients showed significantly modality effect in verbal dichotic listening task, with superiority of right ear (p<005), more important in global cognitively deteriorated patients (p<001). RRMS patients showed a global decrease of reaction time in crossed matching visual and Poffenberger tasks (p<001), without interaction’s effect. Positive correlation was observed between verbal dichotic listening task and both for anterior and posterior body of CC and crossed tactile finger localization task and splenium. Larger callosal area was associated to best speed processing information, only in task with flexibility or inhibition (p<0.05). No difference was found in CC morphological structure between patients with or without global cognitive deterioration. RELEVANCE: These findings suggest the potential clinical value of callosal involvement in MS and underline the interest of interhemispheric assessment in neuropsychological or global disabilty evaluation of the disease.
