T his chapter focuses on the exercise pathophysiology and patterns of exercise responses during CPET in patients with pulmonary hypertension and increased pulmonary vascular resistance, without significant left heart disease (group 2) or a significant obstructive or restrictive ventilatory defect (group 3). Although dyspnoea and exercise intolerance are common but nonspecific symptoms of pulmonary vascular disease, CPET may reveal abnormal exercise patterns suggestive of pulmonary vascular disease in patients with undifferentiated dyspnoea.
