Enhanced spatial realism of sound boosts cognition in DOC patients

Neuroscientific studies often use headphones. However, headphones deminish ecological validity of the sounds as they give the impression of a sound source inside the head. We have previously shown, within healthy participants, that a convolution of the sound with a binaural room impulse response, enhances early discrimination between relevant and irrelevant sounds as observed with evoked potentials measured via EEG. This externalisation of the sound furthermore causes more profound processing of irrelevant sounds. The convolution showed activations in the auditory spatial processing pathway and was thus probably associated with an enhanced representation of the peripersonal space. In patients with disorders of consciousness (DOC), convolution of sounds increased the probability to evoke a cognitive discriminative response for relevant sounds. Thus, the enhanced realism of the spatialised sounds might help in ?calling attention to the outside world?. Here we will compare a larger group of patients with the same paradigm in EEG to explore the effects of convolved sounds on the exploration of patient outcome. We expect that the more realistic stimuli, resulting in enhanced brain processing, will lead to more sensitive patient assessment.
