Prisoners & Patients: Healthcare strategies in a restricted environment

Résumé
This article focuses on three reasons which limit the emergence of a 'contemporary patient' in the sanitary units of prisons: the inmates/patients do not make appointments with healthcare professionals but instead are summoned by them, they are denied access to certain types of documents and information, and they can only count on limited social support to elaborate their view of their illness. Nevertheless, this constraining and disempowering system rarely leads to a disengagement from medical services. Instead, most of the sick inmates take the opportunity, during their incarceration, to seek support. This article aims to show that the motivation of their personal involvement is not necessarily to seek care but sometimes to be part of what Goffman calls a 'privilege system'.
Cet article met en avant trois raisons qui limitent l'émergence d'un « patient contemporain » dans les unités sanitaires en milieu carcéral : les détenus/patients ne prennent pas rendez-vous avec un soignant mais sont convoqués, certains documents et informations leur sont dissimulés et ils ne peuvent compter que sur un faible soutien social pour construire leur rapport à la maladie. Néanmoins, cette organisation contraignante et déresponsabilisante ne produit que rarement un éloignement des services médicaux. Au contraire, la plupart des personnes détenues et malades rencontrées se saisissent de leur incarcération pour s'investir dans une prise en charge. Cet article montre que l'implication personnelle des détenus n'est pas nécessairement motivée par la recherche de soins mais parfois davantage par la volonté de s'intégrer dans ce que Goffman appelle un "système de privilèges".
